Equipment Request Form

In order to check out BCTV video equipment, producers may request equipment in advance either by filling out the following form or by calling the BCTV office. Check the Field Equipment List for more details about the equipment that can be signed out.

Please note:  BCTV does not guarantee that equipment requested will be available. You must be a certified BCTV member to check out equipment. Third parties may not operate equipment even under your supervision. Equipment must be returned on or before the time specified upon check out. Please refer to BCTV’s Policies and Procedures Manual for more information.
